A great original Treasury Department issued order form for Opium, Coca Leaves, Opiates, etc.! An older form was recently shown on the History Channel during a program on illegal drugs. Please see photos!
The form has great graphics with ornate borders, some Treasury seals and lots of stars! It was issued to Beechtold’s Pharmacy in Columbus, OH. An order for different types of Codeine, Empirin, Demerol and Morphine was placed on July 30, 1957.  The top original was sent to Latimer and Kauffman – Lattimer Co., also in Columbus, to fill.
The form measures 7 by 4.5 and is in excellent condition…great in person!
